Terms of Service

Welcome
SignalForm is a form builder operated by MPV Labs LLC("we," "us," or "our"). By using SignalForm, you agree to these terms. If you don't agree, please don't use the service.
We've written these terms in plain English because we believe you shouldn't need a lawyer to understand them.
Your Account
To use SignalForm, you need an account. Here's what we expect:
- You must be at least 18 years old to create an account.
- You're responsible for keeping your login credentials secure.
- Accounts are for individual use — don't share your credentials with others.
- Provide accurate information when you sign up.
- Let us know right away if you think someone has accessed your account without permission.
We can suspend or terminate accounts that violate these terms. We'll tell you why if we do.
Acceptable Use
Use SignalForm for legitimate purposes. Don't use it to:
- Break any laws or regulations.
- Collect sensitive data like Social Security numbers, health information, or financial account numbers through forms unless you have appropriate compliance measures in place. SignalForm is not HIPAA, PCI, or SOC 2 compliant.
- Create forms for phishing, spam, or deceptive purposes.
- Attempt to reverse-engineer, scrape, or abuse the service or its API.
- Resell access to SignalForm without our written permission.
- Interfere with or disrupt the service or its infrastructure.
Fair Use
"Unlimited" on the Pro plan means unlimited for normal commercial and personal use. It doesn't mean unlimited for purposes that place extraordinary load on our infrastructure or that are inconsistent with typical form-builder usage.
Here are the soft limits:
- Responses: Pro includes unlimited responses for standard use. If a single account consistently receives more than 50,000 responses per month, we may reach out to discuss your usage and potentially move you to a custom plan. We will always contact you before taking any action.
- AI form generation: Pro includes unlimited AI-powered form generation. We reserve the right to apply reasonable rate limits (currently no more than 100 generations per day) to keep things running smoothly for everyone. We don't expect anyone to hit this in practice.
- Storage: Form data and responses are retained indefinitely while your account is active. We reserve the right to set reasonable storage limits in the future with advance notice.
- Embeds and bandwidth: No limits on embed impressions or bandwidth for Pro users. If a single form consistently generates more than 1 million impressions per month, we may work with you on a custom arrangement.
The key principle: we want to be generous. These limits exist to prevent abuse, not to nickel-and-dime customers. We will always contact you before restricting anything, and we'll work with you to find a solution.

Payment and Billing
- Pro plans are billed monthly ($15) or annually ($144).
- All payments are processed by Stripe. We never see or store your card numbers.
- You can cancel anytime. Your access continues through the end of your current billing period.
- We don't offer refunds for partial months, but we'll consider refund requests on a case-by-case basis. We're humans, not robots.
- If a payment fails, we'll try to collect for 7 days before downgrading your account to the Free plan.
- We may change our pricing with 30 days' notice. Price changes won't apply to your current billing period.
Your Content and Data
You own your forms and the response data you collect. That's your stuff, not ours.
By using SignalForm, you grant us a limited license to host, store, and display your forms and process your data as needed to provide the service. We won't use your content for anything else.
Here's what we commit to:
- We don't sell your data or your respondents' data.
- We don't use form response data to train AI models.
- When you use AI form generation, your prompt is sent to the Anthropic Claude API. Per Anthropic's API terms, API inputs are not used for model training.

Intellectual Property
We own the SignalForm platform, brand, code, and documentation. You may not copy, modify, distribute, or reverse-engineer any part of the service.
Forms on the Free plan display a "Powered by SignalForm" badge. This badge is part of the free tier terms and may not be removed or hidden. Upgrading to Pro removes the badge.
Service Availability
We aim for high availability but we don't guarantee specific uptime percentages. We're a small team building something we care about, and sometimes things break.
When something goes wrong, we'll communicate openly about what happened and what we're doing to fix it. We reserve the right to modify or discontinue features with reasonable notice.
Limitation of Liability
SignalForm is provided "as is." We do our best to keep things running smoothly — including daily backups — but we can't guarantee perfection.
We're not liable for indirect, incidental, or consequential damages. Our total liability to you is limited to the fees you've paid us in the 12 months before the claim arose.
You agree to indemnify and hold harmless MPV Labs LLC from any claims, damages, or expenses arising from your use of the service or violation of these terms.
Termination
You can delete your account anytime from your account settings.
We can terminate accounts that violate these terms. If we do, we'll tell you why.
After termination, we'll keep your data available for export for 30 days. After that, it will be permanently deleted.
Changes to These Terms
We may update these terms from time to time. We'll notify users of material changes, and we'll update the "Last updated" date at the top of this page.
Continued use of SignalForm after changes take effect means you accept the revised terms.
Governing Law
These terms are governed by the laws of the State of New York, without regard to its conflict-of-law provisions. Any disputes will be resolved in the courts located in New York.
